Attain and colleagues at Dartford, Gravesham & Swanley Health & Care Partnership and Dartford & Gravesham NHS Trust have been shortlisted for an HSJ Partnership award.

Attain and NHS colleagues have worked together to design and mobilise the investment of some of the hospital’s budget in community services.

Investment is focused on third sector dementia support in ED, community teams in the hospital and Hospital at Home and virtual ward and care coordination capacity in the community. The goal is to improve care, reduce the number of admissions and reduce length of stay. We are excited to see the results of these innovative initiatives as they are delivered through the winter.
