Neighbourhood health delivered at Place is a key component of Fit for the Future: Ten Year Health Plan for England. Attain were engaged by the Hull Health & Care Partnership to provide support to the development of their priority ambitions, including the implementation of Integrated Neighbourhood Teams (INTs).

We provided strategic advisory and programme leadership which has helped to deliver:

  • A maturity assessment of the HCP – bringing together partners from across the NHS, Council, and VCSE to assess current the state and agree a baseline from which to develop the functional components of integration (e.g. governance).
  • Confirmation of HCP strategic priorities – we adopted a data driven approach to agree the implementation of Integrated Neighbourhood Teams, Workforce Development, and tackling Health and Care Inequalities as strategic priorities.
  • INT programme delivery – we led the implementation of INTs across Hull which are now delivering integrated support to people with COPD. There is a clear plan in place to scale to other groups of people with complex needs.
  • Stronger governance – we supported the establishment of a refreshed MOU between that provides for delegation to the HCP from both the Hull City Council and ICB.

This work has strengthened Hull’s foundations for integrated, place-based care and set a clear path for future neighbourhood delivery.
